Executive Urgent Care’s Privacy Policy

 

This notice describes how protected health information about you may be used and disclosed and how you can get access to this information. PLEASE REVIEW THIS NOTICE CAREFULLY. Our staff is committed to protecting your health information, which is a right you have and one detailed in the federal Health Insurance Portability and Accountability Act (HIPAA) of 1996.

 

Although Executive Urgent Care has used reasonable precautions to safeguard the confidentiality of information received and sent over the Internet or by electronic mail (e-mail), we cannot guarantee the confidentiality of such information. Executive Urgent Care cannot guarantee that we will receive, review or respond to information you send to us over the Internet or by e-mail. Individuals corresponding with us in this manner waive claims regarding any third party’s access to, or use of, confidential information.

 

In addition to information provided by visitors to our website, we use the normal Internet tracking tools associated with standard Internet protocols and Web based systems. This system information is typically stored in log files and the information is used for aggregate reporting. Aggregate reporting includes: total number of visitors, most visited sections, peak traffic times, etc. Log files are simply transaction records web servers maintain. Those logs are used for recording information, such as:

This California Consumer Privacy Statement (the “Statement”) supplements the Executive Urgent Care Privacy Policy.  It applies solely to California consumers and addresses personal information we collect online and offline. This Statement does not apply to Executive Urgent Care team members or job applicants. Further, this Statement does not apply to protected health information (“PHI”) that is collected by a covered entity or business associate governed by the privacy, security, and breach notification rules issued by the United States Department of Health and Human Services, Parts 160 and 164 of Title 45 of the Code of Federal Regulations, established pursuant to the Health Insurance Portability and Accountability Act of 1996 (Public Law 104-191) and the Health Information Technology for Economic and Clinical Health Act (Public Law 111-5). PHI collected by GoHealth is exempt from the California Consumer Privacy Act of 2018 and its implementing regulations (the “CCPA”).


 

This California Consumer Privacy Statement uses certain terms that have the meanings given to them in the CCPA.

Verifying Requests. If you request access to your personal information, we will not be able to process your request. We do not have sufficient data points to verify your identity and there is no reasonable method by which we can verify your identity to the level of certainty required by the CCPA.

Do Not Sell My Personal Information

For deletion of your personal information, we may require you to provide any of the following information in order to delete your email address: name, email address and postal address. These will be used to send you a one-time code or password to confirm that you are the sole consumer associated with that email address. If you designate an authorized agent to make an opt-out of sale or deletion request on your behalf we may require you to provide the authorized agent written permission to do so, and verify your own identity directly with us (as described above).

Additional Information. If you choose to exercise any of your rights under the CCPA, you have the right to not receive discriminatory treatment by us.
